摘要：随着人口老龄化趋势的逐步加快，中国传统的家庭养老模式受到了很大挑战，机构养老也面临尴尬的境地。社区居家养老作为应对人口老龄化问题的一种新型的模式和选择，无疑具有必要性和现实可行性。可社区居家养老模式在现阶段还不是很成熟，还面临着一系列的问题。本文将以笔者实习所在的南京市建邺区健园社区为例，通过分析社区推进的社区居家养老现状和问题，根据社区的实际情况提出自己的建议。

Abstract：With the gradually accelerating trend of an aging population, the mode of the traditional Chinese family pension has been a great challenge, and the aged also face an embarrassing situation. At this time, the community home care as a new model to deal with an aging population and selection, no doubt the necessity and practical feasibility. However, community home care model at this stage is not very mature, but also faces a number of problems. This article will be Jianye District, Nanjing Kin Park community where I practice, for example, by the analysis community to promote community home care status and problems of its own proposals, according to the actual situation of the community.

　　在针对人口老龄化问题上，社区居家养老作为的一种新型的模式和选择，无疑具有必要性和现实可行性。它不仅是当代养老的一种趋势，还得到了政府的大力支持与推广。由于社区居家养老模式刚开始实施，难免遇到操作和贯彻上的各种问题，但只要积极应对，因地制宜，各个社区间取长补短，定能有助于居民社社会。
